One of the most common questions we hear at Dheeran Hair Transplant Clinic, Coimbatore is: “Doctor, for a natural hairline, is the DHI pen method better or the Sapphire method?”
Short answer: neither method decides how natural your hairline looks.
What decides it is the skill of your hair transplant surgeon: how they design the hairline, how they create micro and macro irregularities, and how they place each graft.
“இந்த கேள்வியே தப்பு. நேச்சுரல் ஹேர்லைன் வர்றதுக்கு மெத்தட் முக்கியம் இல்ல, உங்க சர்ஜனோட ஸ்கில் தான் முக்கியம்.” – Dr. Vivin Prasadh
DHI vs Sapphire: what is the actual difference?
Both DHI and Sapphire are FUE-based techniques. Grafts are taken one by one from the donor area at the back of the head. The only real difference is how those grafts are placed into the recipient area.
| DHI (Choi pen) method | Sapphire method | |
|---|---|---|
| How grafts are placed | The graft is loaded into a pen and implanted directly, without making holes first | Tiny slits are made first with a very thin sapphire blade, then grafts are placed into those slits |
| Who it helps | The doctor’s working style and convenience | The doctor’s working style and convenience |
| Does it decide natural results? | No | No |

That is all. The pen or the blade is a tool. A good tool in untrained hands still gives a poor result, and an experienced surgeon can give an excellent result with either.
What actually makes a hairline look natural?
A natural hairline is never a straight, perfect line. Look at anyone who has never lost hair: the front edge is soft, slightly uneven and gradually becomes denser. A skilled surgeon recreates this using two things.

Macro irregularities
These are the larger shapes of the hairline: the gentle curves, the temple points and the slight forward or backward variations along the front. Without them, the hairline looks like it was drawn with a ruler.
Micro irregularities
These are the small details at the very edge: single-hair grafts placed slightly scattered in front, a soft transition zone, and denser grafts behind. This is what makes the hairline look “grown”, not “planted”.
Graft selection, angle and direction
- Single-hair grafts go in the front line, and two or three-hair grafts go behind for density.
- Each graft must follow the natural angle and direction of your existing hair.
- The hairline must suit your age, face shape and how much donor hair you have for the future.
In the video, Dr. Vivin Prasadh shows two hairlines done at other clinics: one with the pen method and one with the Sapphire method. Both look unnatural. The reason is the same in both cases: the micro and macro irregularities were not designed correctly. The method was never the problem.
How to choose the right hair transplant clinic
If you are planning a hair transplant in Coimbatore or anywhere else, ask these questions before you book:
- Who will design my hairline? The surgeon, not a technician, should plan and mark it.
- Can I see this doctor’s own hairline results? Look at real patient before and after photos, especially close-ups of the front edge.
- How many grafts do I really need, and how much donor hair should be saved for later?
- Who does the implantation? Ask how involved the doctor is during the procedure.
Choose the doctor and their results, not the method name on the brochure.

Frequently asked questions
Is DHI better than Sapphire FUE for a natural hairline?
No. Both are placement techniques. A natural hairline depends on the surgeon’s hairline design, micro and macro irregularities and graft placement, not on whether a pen or a sapphire blade is used.
What is the difference between DHI and Sapphire hair transplant?
In DHI, grafts are implanted directly with a pen without making holes first. In Sapphire FUE, tiny slits are first made with a thin sapphire blade and grafts are then placed into them. The result depends on the surgeon’s skill in both cases.
Why do some hair transplants look unnatural?
Usually because the hairline is too straight, too dense at the very front, or the grafts are placed at the wrong angle. Missing micro and macro irregularities is the most common reason.
How do I choose a good hair transplant surgeon?
Check who designs the hairline, look at the doctor’s own close-up hairline results, and ask how involved the doctor is during the procedure. Do not choose a clinic only because of the method name.
